Replacing Certification Cards
|

You may follow the instructions below, or contact NAUI Worldwide HQ at naui.replacement@naui.org
or call our 800 number listed below and ask for the Replacement Desk. ALL requests
for replacement cards must be in writing. Submitting a request for a replacement
card does NOT guarantee that you will receive one.

To obtain a replacement card, send or fax us a clear photocopy of your existing
card (if possible), a clear photocopy of your training logbook pages, or a clear
photocopy of your temporary certification card. Include these along with the Replacement
C-Card Request Form (approx. 70 k). The form is in PDF format. You will need to
download the Adobe Acrobat Reader (free) from Adobe to be able to read and print
the form. If this does not work, you may obtain a form, by contacting NAUI Worldwide
directly. We also need a copy of a photo id (Driver's License, Passport, etc.) and
Payment. We accept Checks, Money Orders, Visa, MasterCard, and American Express.
Replacement cards are $30 (US). Additional charges for rush shipment and other services

In summary, the steps are:
- Download the Replacement C-Card Request Form (approx 70k). Print this form and fill
it out.
- Copy either your Temporary Card, Training Logbook Pages, your Original Card, or
any other supporting documentation. Attach with the replacement request form.
- Copy a Valid photo ID (Passport, Driver's License, etc.) Attach with the replacement
request form.
- Fax or mail the pages from the steps 1-3 above to NAUI Worldwide with payment. Only
Credit Card orders are accepted via Fax. We do not accept orders via email at this
time.
