NAUI Diver Request

Please enter your Diver Certification information below. The Customer Service Department will assist in verifing your records.

In order to be added to the NAUI Diver Lookup, all Diver Certification records
must be verified.

If you have proof of your certification, for example: c-card, wall certificate, signed logbook pages, attach images (approximately 2mb in size in JPG format) to an email, and provide your date of birth, the city at time of certification, and send to nauistucert@naui.org to expediate your request.

This tool is to be used for verification purposes only, it is not for replacing your card. Cards can only be replaced by completing a replacement card request.
